Appointment Process:
The first step is to send an email to luxtelepsych@gmail.com with the following information:
Your Full, Legal Name, including middle names
*Your preferred name, if different from your legal nameDate of Birth
Email Address
Health Insurance Company
Cell Phone number (text capable)
Written permission to send you texts and emails regarding psychiatric information through osmind.
Example: "I, (full name), consent to Dr. Jessica Jawidzik contacting me via text message and email for psychiatric purposes."
Unfortunately, our platform requires both methods of communication, but please feel free to let us know your preference if you have one!
Once I have received this information, you will receive an invitation via text and email to make an osmind account, which is HIPAA Protected. From there, we will continue to communicate through osmind for your privacy and safety.

Due to the high volume of referrals that we receive, we cannot respond to every new patient request. If you do not hear back from us within 4-5 days, please assume that we cannot provide services to you at this time.
